Commonwealth Aged Care Packages

Commonwealth Aged Care Packages (CACP) provide case management to people over 65 years of age who have been assessed as requiring low level aged care services, but who have chosen to live in their own home with support.

The aim of the CACP program is to assist residents to remain at home by providing coordinated, flexible care services that promote personal choice, independence, dignity and safety.
 
Case management provides support and advice to coordinate care, advocacy to assist residents to access support to meet their care needs, information and education about available services and the service system and encourage residents to feel empowered to make decisions about their needs.

Case managers work closely with residents and their carers, families and health professionals, to plan and coordinate support and to encourage independence. The types of services provided may include: 

  • Personal care
  • Preparation of meals
  • Medication prompting
  • Transport to shops, appointments and social gatherings
  • Household tasks such as basic house work and shopping
  • Home maintenance related to safety e.g. installation of hand rails and maintenance of smoke detectors  

How to apply for a Community Aged Care Package?

Access to this service is different to the access steps to other Council services. If you need support or more information about this service, contact the Aged and Disability Services Department on 9262 6110.
  
Residents will need an assessment from the local Aged Care Assessment Service (ACAS) located at the Peter James Centre in Forest Hill. Contact the service by calling 9881 1875 or email acas@peterjames.org.au

The ACAS refers eligible clients to a CACP program. Residents are placed on a prioritised waiting list and are contacted by a Community Aged Care Provider as soon as a vacancy occurs.

Residents who are already on the waiting list for a Whitehorse City Council CACP can contact Aged and Disability Services directly on 9262 6110.
