From 27 February 2017, funding for a Home Care Package follows you, the consumer.

This will allow you to choose a provider to administer your package funding. You are able to change providers, if you wish, at any time.

We believe older people should be valued and respected, connected to their communities and in control of their lives. Our support team works closely with you to individually tailor and coordinate a package of care that will support your lifestyle at home. 

Qualified Case Managers - Dedicated professionals servicing your needs, providing peace of mind to you and your family

Whitehorse City Council Home Care Packages will support you to remain living at home, while providing choice and flexibility in the way care and support is provided.  You will have access to a Case Manager who will work with you to understand your needs and individually tailor a care plan which is monitored and reviewed with you regularly. All support and services are planned and arranged with you, based on your individual needs and what’s important to you.

Access to Whitehorse City Council Home Support staff - familiar, experienced and dedicated staff that will provide you with continuity of care

Whitehorse City Council Home Care Package consumers can choose to have services provided directly by Council’s Home Support staff, or from a range of external approved service providers. Only Whitehorse administered packages have access to our highly-trained in-house support staff.  We ensure care is consistent and of the highest standard, while remaining flexible to adapt to your individual needs. We ensure all external providers comply with program guidelines, supplementary Council requirements and national standards.

How do I access a Whitehorse City Council Home Care Package?

Before commencing a Home Care Package with the City of Whitehorse there are a few important things to do and consider:

  1. An Aged Care assessment completed by an Aged Care Assessment Service (ACAS) is required to determine your eligibility – you can arrange a free assessment via the My Aged Care website or contacting 1800 200 422
  2. At your assessment you need to advise the ACAS representative that you wish to nominate City of Whitehorse to be your Home Care Package Provider. 
  3. Alternatively, when you are notified that you have been assigned a Home Care Package, simply contact City of Whitehorse and provide your unique referral code. We will arrange a time to meet and work with you in tailoring your requirements.
