Council's Home and Community Care Department provides support and services to:
- Older people who are frail or at risk
- People with a disability
- Carers of older people or people with a disability
Services are provided to help people remain living at home and to continue to be actively involved in community and social life. Services include:
How Can These Services Assist?
To find out if these services can assist you or someone you know living in the City of Whitehorse, an assessment by one of Council’s assessment officers needs to be arranged.
To arrange an assessment, a referral can be made by telephone, email, letter or fax to Council’s Intake Officer. A friend, neighbour, doctor or relative can make a referral, or anyone concerned for the wellbeing of a resident, with the person's consent. Residents can also refer themselves directly.
The assessment is a conversation to discuss your needs and provide you with advice and information, including details about Council and other services that may be available to assist and support you. At this discussion the assessment officer will also fill out a form that requires your personal, financial and other relevant details. At the end of this discussion you will be aware of information, services and supports that may be available to you.
